The following is a list of items required to work for Alpine Painting:
- Signed Subcontract Agreement or Per Job Subcontract Agreement
- Certificate of Insurance as indicated in the Subcontract Agreement and in the Sample Certificate attached. (Alpine must be added as additional insured)
- Completed W-9 form
- Business Registration Certificate
-All licenses required for the state where the job is being performed
- References and Job Pictures
- Public Works Certificate (if you are to perform Work on Public Projects)
-Application for Public Works Contractor Registration
- One Hour Meeting with Operations Manager and Sales Director to Discuss Capabilities and past job experience
